Step-by-step Procedure of the Breast Augmentation

1.What is the difference between breast augmentation and breast lifting? How do I know which one I need?

In order to determine the type of the surgery, we need to consider different factors such as your age, breastfeeding, pregnancy, genetic predisposition, possible sagging in the breast which happens over time. Also when you come to the appointment the following parameters are evaluated such as the distance of the nipple to the collarbone, the length of the chest circumference, the width of the shoulders, your height and your weight. Based on all these factors the type of the surgery you need is determined. 

2.How do we plan the surgery?

According to the measurements and drawings made before the surgery, we would have 4 main topics to be defined: 
Incision place: where to place the breast implant. The implant can be placed with a 1.5 cm incision under the breast, with an incision from the nipple or with an incision under the armpit. In order to decide on the incision place we take into account the age of the patient, breastfeeding experience, tendency to keloid (bad scar) and social life of the patient. 
Implant place: where to place the implant. The implant can be placed in roughly 2 positions: the sub-muscular position or the super- muscular position. The under muscular position is now classified as dual-plane 1-2-3. In cases where the patient's chest tissue thickness is medium or good, a sub-muscular placing may be preferred. However, in cases where chest tissue is inadequate, a dual-plane is preferred. In our routine practice, we mostly prefer a dual-plane because it looks more natural and hides the implant lines better. 

Implant shape. There are 2 basic implant shapes: anatomical (teardrop) and round. Based on the patient's chest structure, weight, shoulder width, and desired chest shape we will have the guidance in the selection of implants. I always ask my patients the following questions during this selection: If the cleavage is the key point for you, if you no longer want to use padded bras and want breasts with top fullness even without using a bra, it’s better to choose a round implant; If naturalness is at the forefront for you, choose a teardrop implant if you imagine breasts that are lightly ptotic and don’t look like being operated when naked. 

Implant size. In the preoperative drawing, many parameters including the dimensions of the rib cage, shoulder width, waist width, height and weight can be calculated and the patient can be told an implant size range. For example, minimum 250cc and maximum 350cc implant can be placed. Based on the previous examples (to be shown) of patient’s results who already had the surgery, we will identify which size is closer to the patient's will together. 

3.Is there a lot of pain after the surgery, when can I return to my daily life?

You stay 1 night in the hospital after the surgery. At the end of the surgery, we put a new bra on you and you should not remove your bra within 1 week. Therefore, it is not possible to take a shower for 1 week. Some patients have drainage that is withdrawn after 24 hours. By this time the pain is almost gone, but serious rest is required for a week. Because even in light exercises such as standing up and walking during the first week, there might be a feeling of pain and tension in the chest. This pain may be slightly higher in surgeries where the sub-muscular position is preferred. After a week, all kinds of daily activities can be returned, except for heavy lifting. 

4.What can be done to reduce the pain after surgery?

In some cases intercostal nerve block can be performed under ultrasound after surgery. Thanks to this block, patients can overcome this week much more comfortably. If the patient has rest during the 1st week of postoperative period, the likelihood of pain will not be very high. 

5.Will this surgery prevent me from breastfeeding in the future? Will there be numbness in my nipples?

Breast enhancement surgery will definitely not prevent you from breastfeeding, this surgery has nothing to do with the milk canals or glands. If a nipple was selected as a place of putting the implant, you may experience a 10-20% loss of sensation in your nipple as a low probability. Therefore, in our routine practice, we prefer the incision under the breast for each patient under the age of 40 

6.When can I start sports, swim and sunbathe after surgery?

In 1 week after the surgery light sports can be started, including walking, yoga and some pilates movements. In 1 month after the surgery all kinds of light-heavy sports, swimming, diving can be done. You can sunbathe with a protective bra already in 1 week after the surgery. 

7.Do I need to replace my breast implant in the future? What is capsule formation?

Most of the new generation implants are lifetime, they may need to be replaced for two reasons: the first is when you want to enlarge or shrink your implant; the second is when there is a capsule formation around the implant. Capsule formation is the case of a sheath surrounding the implant, which can occur in 1-2 patients per hundred thousand, after an average of 5-10 years post surgery and distorts the shape of the implant. Numerous scientific studies have been conducted on the causes of capsule formation, some of which can be counted as the choice of rough surfaced implants, post-operative blood accumulation (hematoma), infection. In such cases the implant should be removed, the capsule should be cleaned and a new implant should be placed. Capsule formation is a very rare case, especially if all measures are taken to avoid it the possibility is almost zero. 

8.Is it true that implants cause breast cancer

Not true. Although lymphoma formation has been observed around the sheath of some implants, this has turned out to be related to the implant structure and these implants have been discontinued. 

9.Which brand of implant do you use?

I am currently using Mentor and Motiva brands implants. After explaining the advantages and disadvantages of both brands to our patients, we make a joint decision which one to choose. Both brands provide a lifetime warranty. 

10.Do you have any package solutions?
